There are a few simple steps first
Before we start chiseling away at that body of yours, we have some simple forms that we need you to fill out and/or print:
- Client Intake Form – Please fill out, print, sign and deliver to trainer
- Policies & Procedures – Please print, sign and deliver to trainer
- The PAR-Q – Choose applicable version: Standard, Medical Condition, or Pregnant. Please print, sign, and deliver to trainer
